Macroscopic appearances

The tumour is multilocular, the locules being filled with creamy material.

There are multiple cysts lined by bland stratified squamous cells with orthokeratosis and parakeratosis. The lumina are filled with laminated keratin. Ruptured cysts induce a foreign body giant cell reaction. A basement membrane separates the cyst lining from the fibrotic stroma. There are also solid nests of squamous cells.
